Colorado Medical Marijuana (MMJ) Registry Identification Card

The Colorado medical marijuana registry card is the document issued by the Colorado state health agency that authorizes patients to legally use medical marijuana.

To qualify for a medical marijuana registry card, you must:

•submit an application and pay a $90 fee (which is non-refundable, even if you’re denied the card)

•reside in the state of Colorado

•submit a completed application form to:

•provide written documentation by a physician and copies of your medical records, proving that you have a debilitating medical condition that could be helped by medical marijuana

In your medical marijuana registry application, you’ll also need to include your name, address, date of birth, social security number, as well as the name, address, and phone number of the physician who provided the medical marijuana referral. Information in the medical marijuana registry that the state maintains is confidential.

From the time you apply for a medical marijuana registration card, you’ll be approved or denied within 35 days.

If, for some reason, your medical marijuana application is denied, you have to wait at least six months to reapply.

If you no longer have a debilitating medical condition, you’re required to return your registry identification card to the state health agency within 24 hours.
